Who would have thought that listening would be a skill necessary for home care? Initially we had no clue. Well Bridges 2 Life Home Care is here to help you and your family because we listened. By listening we learned that helping people was something we do well. We’ve been told our compassion and commitment to excellence has made an impact in people’s lives. Well we plan to bring that same listening attitude to your loved ones. We don’t pretend to know everything, but we do know how to listen, show compassion, and provide excellent care.

At Bridges 2 Life Home Care one size doesn't fit all when it comes to excellence in care. Meal preparation, hygiene, cleaning, and supervision could be daily services among others, we provide. Individualized care plans that fit your specific needs will be developed strategically, relationally, and resourcefully. We want your home care experience to be something you and your family will enjoy!

Steps to Care

INITIAL CALL - Once we’ve received your information, we will have a detailed call to gather more information on the care situation, answer any pressing concerns, and set up a time to meet in person.

IN-HOME CARE ASSESSMENT - Here we will answer all your questions, and ask some of our own to craft a Cared-4℠ Customized Care Plan to your unique needs.

START OF CARE - We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.

CLIENT VISITS - Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.

Hands-on bathing or assistance with tub or shower.

Assistance with dressing, toileting, and ambulation.

Catheter care, but not insertion.

Personal care services

Hands-on assistance with activities of daily living and instrumental activities of daily living, when incidental to assistance with activities of daily living.

Assistance managing the individual's home and handling personal affairs.

Assistance with self-administration of medications.

Homemaker services such as cleaning, mail, and laundry.

Respite services for the individual’s caregiver.

Errands completed outside of the presence of the individual such as picking up prescriptions and groceries.
